FIELD REGISTERED NURSE

 Coordinates and supervises patient care to ensure continuity when patients are referred from referral sources.  May be responsible for case management and provides skilled nursing services to patients in their homes.  Responsible for recording clinical and progress notes.  This position works under the direct supervision of the Director of Clinical Services.

 Individual must be a Registered Nurse with one (1) year of acute care clinical experience within the  last three years in a home health agency, home care setting, or health care facility.  Must meet educational experience requirement outlined in the laws of the State of California.  Minimum one (1) year clinical experience in delivery of infusion therapy; prefer two (2) years.

 Ability to work flexible hours as required to meet identified patient needs.

Must be currently licensed in California.

HOME HEALTH AIDE

The Home Health Aid is responsible for providing various patient related services, meeting the personal care needs and promoting comfort and independence to acutely and/or chronically ill patients in their homes.  Responsible for recording clinical and progress notes.  This position works under the direction of the Registered Nurse.
 
Completion of state approved Home Health Aid training program and/or successful passing of a competency evaluation program which meets state and federal guidelines.


LICENSED VOCATION NURSE

The Licensed Vocational Nurse is responsible for providing direct nursing care as prescribed by the physician to acutely ill or chronically ill patients in their homes.  Responsible for recording  clinical progress notes and coordinating patient care activities/services to which he/she is assigned.  This position works under the direction of the Registered Nurse and/or designated supervisor.

This individual must be a graduate from an approved school of vocational nursing and meet educational experience requirements as outlined in the laws of the State of California.

A minimum of one (1) year of acute care experience preferred; previous home health experience preferred.

Must be currently licensed in California.


MEDICAL SOCIAL WORKER

The Medical Social Worker is responsible for providing social case work and social histories to acutely and/or chronically ill patients in their home.  Responsible for recording clinical and progress notes and coordinating other patient care activities/services to which he/she is assigned.  This position works under the direction of the Director of Clinical Services.

The individual must have graduated from an approved school of social work with a Master’s Degree or is a Licensed Clinical Social Worker.

A minimum of one (1) year of social work experience required; previous home care experience preferred.

Must be currently licensed in California.



OCCUPATIONAL THERAPIST

The Occupational Therapist is a qualified professional who directs supervises, evaluates, and provides occupational therapy services to patients in their home, as prescribed by the attending physician. This position works under the supervision of the Director of Clinical Services.

He/she is responsible for providing skilled therapy care to acutely and/or chronically ill patients in their homes; recording clinical and progress notes and coordinating other patient care activities/services to which he/she is assigned.  

The individual must have graduated from an occupational therapy school approved by the Council of Medical Education of the AMA, in collaboration with the American Association of Occupational Therapy.  

A minimum of two (2) years of acute care experience required; previous home care experience preferred. 

Must be currently licensed in California and registered by the Occupational Therapy Association.





PHYSICAL THERAPIST

The Physical Therapist is a qualified professional who directs supervises, evaluates, and provide physical therapy services to patients in their home, as prescribed by the attending physician.  This position works under the supervision of the Director of Clinical Services.

He/she is responsible for providing skilled therapy care to acutely and/or chronically ill patients in their homes; responsible recording clinical and progress notes and coordinating other patient care activities/services to which he/she is assigned.  

The individual must have graduated from a physical therapy school approved by the Council of Medical Education of the AMA in collaboration with the American Physical Therapy Association or its equivalent.

A minimum of two (2) years of acute care experience required; previous home care experience preferred. 

Must be currently licensed in California.







SPEECH LANGUAGE PATHOLOGIST

The Speech Therapist will direct, supervise, evaluate, and provide speech and language therapy services, as prescribed by the attending physician.  He/she is responsible for providing skilled therapy care to acutely ill and/or chronically ill patients in their homes.  Responsible for recording clinical and progress notes.  Coordinates other patient care activities/services to which he/she is assigned.  This position works under the supervision of the Director of Clinical Services.

The Speech Therapist must be a member of the American Speech Language Hearing Association with a certificate of clinical competence in the appropriate area.

A Master’s degree in Speech Language Pathology in Audiology.  One (1) full year of supervised work experience.

A minimum of one (1) year of acute care experience required.  Previous home care experience preferred.

Must be currently licensed in California.
